Patient Navigator
Rutgers University – New Jersey Medical School (Department of Medicine – Infectious Disease) is seeking a Patient Navigator to provide clinic and community-based HIV/STI testing, and case management services to persons with and at risk for HIV infection.
The position requires performing a biopsychosocial assessment, developing informed care plans addressing social service and treatment adherence barriers, and maintaining confidential integrity of all client information. The Patient Navigator will work in clinical settings and on a mobile health unit, including selected weekend and evening hours, and must proactively report issues requiring action to the Program Manager.
Key Responsibilities
Provide confidential assessments for clients living with HIV and those referred for PrEP, determining their HIV prevention needs and registering and planning prevention services.
Deliver HIV and STI confidential pre‑ and post‑test counseling in accordance with NJ Department of Health, Senior Services, and CDC guidelines, documenting all interactions.
Coordinate PrEP and PEP services for clients.
Provide medical case management for newly diagnosed HIV patients.
Maintain documentation of all client encounters and fulfill reporting requirements per grant and department standards.
Work in clinical settings and a mobile health unit, including select weekend and evening hours.
Maintain client confidentiality and report issues to the Program Manager in a timely manner.
Preferred Qualifications
Five (5) years full‑time professional experience with HIV/AIDS case management.
Bilingual proficiency (English/Spanish).
