7am-3pm; 3pm-11pm; 11pm-7am

The ideal LPN candidate will:

Be a graduate of an approved school of nursing

Have an active LPN license in the State of New York

Hold CPR certification

Have the ability to observe and identify residents' needs, report to the RN/Supervisor, and collaborate with the RN in making clinical decisions

Prioritize and coordinate activities of the nursing unit

Be an excellent problem solver with strong decision-making skills

Demonstrate leadership abilities

Prior experience in a skilled nursing facility is preferred but not required

Experience with SigmaCare is preferred but not required

About the position: The LPN functions under the supervision of the RN/charge nurse and the Nursing Supervisor. Responsibilities include, but are not limited to:

Leading and coordinating resident care in collaboration with CNAs, physicians, and other disciplines, including assessment and family involvement

Planning and implementing care, documenting thoroughly in the EMR

Proficiency or ongoing training in wound care, ostomy care, Foley catheter care, medication administration (including G-tube and IVs), trach care, and suctioning

Monitoring for changes in condition and responding promptly to prevent re-hospitalizations and emergencies, responding efficiently and calmly during emergency situations
