Responsibilities The Speech Therapist in Home Health is responsible for assessing and evaluating patient care needs related to treating speech and language disorders, and functional training in communication, swallowing, and cognitive impairments. Based on this assessment, the therapist works to determine a treatment plan, performs interventions to improve the patient's well-being, and evaluates progress.
Provides services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other healthcare team members.
Evaluates the patient's functional level using diagnostic and prognostic tests, and assists in developing the therapy plan of care.
Treats patients to communicate effectively, expressing thoughts according to their condition and accepted standards of practice.
Observes, records, and reports to the supervising nurse and/or physician the patient's response to treatment and any changes in condition.
Instructs the patient, family, caregiver, and healthcare team in speech therapy areas where they can participate.
Education and Experience License Requirements
Must be currently licensed in Speech Therapy in the state of practice.
Current CPR certification.
Must have a valid driver's license, vehicle insurance, and access to a dependable vehicle or reliable transportation.
