Care Coordinator, Care Management

Responsibilities
Assess patients by screening for potential discharge needs
Meet directly with patient/family to assess needs and develop individualized care plan
Facilitate communication and coordination among health care team
Maintain current information of community resources and refer patients appropriately
Work collaboratively with multidisciplinary and post‑acute care teams for timely transitions
Develop discharge plan and ensure it meets continuing care needs
Document and communicate information in medical record
Participate in multidisciplinary rounds and committees
Identify and refer quality issues to Quality Management Program
Perform reassessments against care goals and revise plans as needed
Provide patients/families with discharge options and educational resources
Provide appropriate CMS documents per regulatory guidelines
Utilize social determinants of health screening tools during intake
Collaborate on crisis intervention, counseling, abuse reporting, guardianship, psychosocial assessments, observation management, capacity management, hospital throughput
Coordinate referrals to relevant facilities and services
Maintain annual competencies and training in applicable platforms
Other duties and/or projects as assigned
Adhere to HMH organizational competencies and standards of behavior
Qualifications
Bachelor's Degree in Nursing or related field, or Master's Degree in Social Work
Effective decision‑making skills and influential leadership
Excellent verbal, written and presentation skills
Moderate to expert computer skills (Microsoft Office, Google Suite)
Familiar with hospital resources, community resources, utilization management
Proficient in computer platforms
Preferred Qualifications
Master's degree
Licenses & Certifications
Required: NJ State Professional Registered Nurse License OR NJ Licensed Social Worker OR NJ Licensed Clinical Social Worker
Preferred: Care Management certification by a nationally recognized organization

In addition to full‑time and part‑time (20+ hours/week) positions, HMH offers a comprehensive benefits package, including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits. Minimum annual salary $90,750.40.
