Overview
Physical Therapist - In Motion Eagle Harbor at Bon Secours Mercy Health. The Physical Therapist completes initial assessments, ongoing assessments and provides skilled therapeutic interventions to patients across settings including outpatients, inpatients, pediatrics and off-site locations as part of the continuum of professional care. Responsibilities
Evaluates patients, gathers pertinent information from various sources, and develops an appropriate treatment plan with specific goals. Responsible for ongoing assessment, updating the plan as needed. Provides direct therapy to patients daily following the patient ??s plan of care and current standards of practice for Physical Therapy. Provides instruction in therapeutic procedures to patients and family. Maintains accurate and concise records of treatment provided and patient progress. Documents all care, coordination of services, orders, goals, etc. in the patient ??s electronic medical record in an accurate and timely manner. Provides timely co-signature for supervised physical therapy assistants, if applicable. Maintains productivity at a level that supports quality care and meets department standards. Follows departmental policies and procedures regarding safety, infection control, time off requests and timecard recording; completes required education and compliance requirements. Supervises the physical therapy assistant ??s treatment interventions and professional growth. May precept or evaluate students on clinical experiences/internships from participating colleges or universities. Promotes rehabilitation services and wellness center activities with patients, families and caregivers, when applicable. Coordinates with other disciplines, agencies/companies, families, etc. to ensure patient needs are met. Licensed Physical Therapist in the state of current practice or eligible for traineeship or temporary license in the state of current practice (required). BLS Basic Life Support (preferred upon hire; required within 30 days for BSMH, required at hire for RSFH). Work Experience
