St. Patrick Center, a ministry of Catholic Charities, is hiring a Mental Health Specialist to join our Housing, Employment and Recovery Opportunities (HERO) Program. We are looking for a compassionate professional to conduct mental health assessments, provide case management, and support clients facing severe and persistent mental illnesses and chemical dependency challenges. The core work schedule is Monday through Friday, 8:00 am to 4:30 pm.

Overview Working collaboratively with individuals, families, and community partners, the HERO program aims to end chronic homelessness in the St. Louis region through Housing First, removing barriers, conducting intake and assessment early, placing clients in housing, and providing ongoing support to achieve goals.

Responsibilities

Conduct comprehensive, ongoing assessments of participants’ mental illness, physical health, housing status, and other relevant needs.

Provide direct clinical services to clients individually and in groups in office and community settings; adjust treatment plans to support coping with internal and external stresses.

Refer clients to appropriate community agencies and assist with access as needed.

Act as liaison with community agencies to coordinate treatment processes.

Collaborate with case management team during daily/weekly meetings.

Facilitate treatment planning groups and set goals with clients to support recovery and wellness.

Provide on-call crisis intervention in coordination with other case managers, including after-hours, nighttime hours, and weekends.

Follow up on program placements weekly or monthly and document progress.

Complete daily and monthly departmental paperwork; maintain client tracking system and files.

Assist team members with life skills classes; manage a caseload of 5-10 clients.

Respond to after-hour emergencies and participate in budgeting and other supportive services as needed.

Research additional benefits to enhance services and support the care model across St. Patrick Center programs.

Contribute to achieving departmental mental health goals and annual reporting requirements.

Other duties as assigned.

Qualifications

Master’s degree in a related area from an accredited university or college required.

Licensure or credential in Social Work or Counseling required.

Understanding of mental illness and substance use; Trauma-Informed Care training preferred.

Experience with wrap-around services; home visits; knowledge of area/community resources for referrals; knowledge of HIPAA policies.

Driver’s license (MO residents require Class E), reliable vehicle, and valid auto insurance.

Skills

Excellent communication and counseling/crisis intervention skills.

Self-motivated with respect for clients and staff; cultural sensitivity; ability to work in a team; moderate computer skills.
