Compensation & Benefits
Starting Pay: $23.00 per hour

Position Summary The Case Manager provides direct support to youth and families through coordination of services and treatment planning. This position will work with multiple community partners in supporting clients living in low-income housing in the Toledo area. Work will be done in the client's home, in the community and while facilitating groups and providing community-based services.
Key Responsibilities & Role Highlights
Provides Therapeutic Behavioral Services to an assigned caseload of clients in accordance with the State mandates and established Agency and program policies, procedures, philosophy, and client individual service plan.
Documents all services provided and maintains records utilizing the electronic record in accordance with agency policy/procedure.
Maintains client confidentiality and ensures release of information is handled in accordance with federal guidelines and agency policy/procedures.
Participates in team meetings, supervision, in-service training and conferences according to established policies, procedures, and applicable regulations.
Education & Experience Requirements
High School Diploma or GED accepted with relevant work experience.
Associate degree in social work, psychology or related field preferred.
Bachelor's degree preferred.
Prior experience preferred.
Knowledge of accessing behavioral health services in Lucas County.
Must demonstrate knowledge of mental illness, crisis management and the ability to establish and maintain therapeutic relationships with clients.
Must demonstrate the ability to define problems, analyze data, establish facts, and draw valid conclusions and make recommendations.
Must demonstrate positive attitude, commitment to provide superior services and enthusiastic support for the mission of the agency.
Must establish and maintain effective working relationships.
Must demonstrate the ability to represent the agency in a positive manner when in the community.
Must be able to work with ill, disabled, emotionally upset and at times hostile individuals.
