Join Hartford HealthCare HomeCare as a Physical Therapist. The role involves evaluating patients, developing and implementing PT care plans, coordinating with Agency personnel and community resources, and supervising clinical staff. The position supports patient safety, quality care, and appropriate end-of-life goals in hospice/hospice-related home care settings. Responsibilities

Manage a patient caseload on a daily basis. Evaluate patient’s level of function and develop an appropriate care plan under the direction of a physician, collaborating with the Hospice Interdisciplinary Team. Implement and monitor the plan of care to provide safe, efficient, quality care. Provide hospice-related care with appropriate end-of-life goals, emphasizing patient safety and patient/caregiver education. Evaluate and revise the care plan as patient needs change, coordinating with Agency personnel and community resources (e.g., Community Health Nurse, Hospice Nurse, other therapists, social workers, Home Health Aide services, physicians).

Coordinate patient care with Physical Therapy Assistant and supervise their performance; communicate with the PTA daily. Organize and prioritize caseload to ensure patient needs and Agency productivity standards are met. Keep Rehab Coordinator informed regarding weekly caseload and patient issues to support staffing and patient care needs. Understand after-hours procedures and educate patients on on-call coverage. Maintain clinical and reimbursement documentation reflecting Agency policy and quality of care; prepare legible daily records and reports. Develop written patient care goals and outcomes in collaboration with the Health Care Team or Hospice Interdisciplinary Team. Update and submit physician orders in a timely manner. Collect and document necessary financial and reimbursement information and communicate with Agency personnel and reimbursement sources. Obtain prior authorization for services when indicated and maintain timely communication with payors. Complete discharge records within 24 hours after the last patient visit and inform relevant staff promptly. Qualifications

Education & Training:

Graduate of an accredited school with a Bachelor’s of Science Degree in Physical Therapy. License/Certification:

Current Connecticut license to practice Physical Therapy. Experience:

Minimum of one (1) year of clinical experience; three (3) years preferred. Skills: Knowledge of principles and practices of Physical Therapy. Excellent verbal and written communication skills. Strong organizational skills. Excellent interpersonal skills; ability to interact with patients, families, physicians and co-workers. Ability to handle stress and exercise independent judgment.
