RN Field Staff Home Health Case Manager Per Diem

Southwoods Health at Home started in 2022. We hiring a Per Diem RN Case Manager to support our team. The RN Case Manager will see their assigned caseload in patients’ homes from start of care through discharge.

Per Diem.

Essential Duties

Participates as a team member in support of patient outcomes and experiences

Plans care in order to meet the needs of patient safely and effectively

Considers cost containment by using resources appropriately

Maintains patient privacy and ensures confidentiality of protected information

Monitors for changes in patient condition and carries out ordered interventions appropriately

Accurately documents patient condition and responses to delivered interventions

Keeps the multidisciplinary team updated as to progress toward goals of care

Participates in facility trainings and learning opportunities

Ensure all processes at responsible physician practice maintains compliance with all regulatory agencies

Perform other duties as assigned

Qualifications

Active RN license in the state of Ohio

Current AHA BLS certification

Valid Driver’s License and proof of current car insurance

Bachelor of Science in Nursing (preferred)

Critical thinking and problem‑solving

Leadership skills and ability to appropriately delegate

Effective communication skills

Ability to articulately document clinical picture

Prioritization and time management

Maintains professional demeanor at all times
