Home Care Position for Mendota, IL OSF offers an environment for clinicians that is supportive in professional development and offers paid continuing education units, tuition reimbursement for further education, and 401k with $1 per $1 match on first 5% of salary! In addition, OSF Healthcare qualifies for the Public Student Loan Forgiveness program. Expected pay for this position is $45.21 - $53.19/hour. Actual pay will be determined by experience, skills and internal equity. This is an hourly position. OSF will invest in you on day 1! Candidates with 5+ years of direct experience will receive 24 hours of Vacation and Holiday time that will be available for use beginning day 1. This is in addition to our generous time-off benefits. REQUIRED QUALIFICATIONS Education Doctoral degree in Physical Therapy Those licensed after 2016 must have a Doctoral degree. Those who were licensed prior to 2016 are exempt from this requirement as grandfathered under prior IDFPR or MBPT requirement for licensure (which allowed bachelor’s/master’s degree; the normal industry requirement prior to 2016). Licensure/Certifications Respective State license in Physical Therapy American Heart BLS Valid state Driver’s license and Insurance Other Skills/knowledge Demonstrated knowledge and skills necessary to provide patient care appropriate to the population served. Strong interpersonal, written and oral communication skills to communicate plan of care and educate family members on care. Communicates effectively with other treatment team members when gathering and exchanging patient condition information. Analytical ability to evaluate patients\' physical and functional status, establish therapy goals, formulate treatment plans consistent with goals, observe patients and document status, and assist techs and assistants in resolving problems. Excellent computer skills and the ability to process detailed information in an accurate manner.
