Physical Therapist Pediatric Home Health PRN Join to apply for the Physical Therapist Pediatric Home Health PRN role at Vanderbilt University Medical Center (VUMC) in Nashville, Tennessee. This PRN, pediatric home health position provides one-on-one, meaningful care to children in their homes across the Greater Nashville area with a flexible weekday schedule. Responsibilities
Deliver skilled physical therapy services to pediatric patients in their homes. Develop and implement individualized care plans in collaboration with families and care teams. Document patient progress and outcomes using EMR systems. Participate in interdisciplinary case reviews and care coordination. Analyze information from assessments to determine rehabilitation diagnoses and plan care accordingly. Collaborate with stakeholders to establish and execute the care plan and coordinate care across the rehabilitation continuum. Apply evidence-based interventions and adjust treatment as needed to meet goals; promote return to home, community, and school/vocational environments. Recognize that responsibilities are a general overview and may include additional duties as assigned. Qualifications
Licensed Physical Therapist in the state of Tennessee. Experience in pediatrics, especially with medically fragile infants and children; acute care or home health experience preferred. Proficient in Electronic Medical Records (EMR) documentation and care coordination. Strong communication and organizational skills. Passion for working with children and families in a home-based setting. CPR certification and valid driver’s license required. Education: Bachelor’s degree. Experience Level: Less than 1 year.
