Rehabilitation Admissions Coordinator
The Rehabilitation Admissions Coordinator performs activities and duties related to pre‑admission evaluation and admission of patients to the inpatient rehabilitation unit. The goal is to identify patients who meet program criteria and who will benefit from an acute rehabilitation program. The coordinator reviews medical records to complete the pre‑admission screening tool, collaborates with MDs and case management to facilitate timely transfer, ensures benefit authorization, determines bed and team assignments with nursing and others, builds relationships with external referral sources, gathers and tracks data for planning, and ensures compliance with regulatory and accreditation organizations.
Responsibilities
Perform pre‑admission evaluation and admission of patients to the inpatient rehabilitation unit.
Review medical records and complete the pre‑admission screening tool.
Collaborate with MDs and case management to facilitate timely transfer.
Ensure benefit authorization is completed as required.
Coordinate bed and team assignments with nursing and other staff.
Build relationships with external referral sources.
Gather and track data to use in planning.
Ensure compliance with regulatory and accreditation requirements.
Minimum Requirements
Two years of experience as an RN.
Associate degree in Nursing.
RN license in AZ or a COMPACT state.
Preferred Requirements
Previous experience in an Admissions Coordinator or Rehabilitation Coordinator role.
Bachelor's Degree in Nursing.

Pay Range $39.18 – $58.28 per hour
