Description St. Mary Home Care & hospice is seeking a contingent Medical Hospice Social Worker. St. Mary’s Home care & Hospice is a member of Trinity Health At Home, a national home care, palliative care and hospice organization serving communities in twelve states. Our family-centered approach includes teaching patients and their families about their care, coordinating additional services, and researching community resources.

Employment Type Part time

Responsibilities The Medical Social Worker is responsible for the provision of consultation and service to homebound individuals in need of assistance with the solution of complex social and emotional problems, which impede management of medical regimen.

Qualifications

Bachelor’s Degree from a school of social work accredited by the Council on Social Work Education.

Currently licensed to practice as a Medical Social Worker as applicable for Georgia.

One (1) year of social work experience in health care setting.

Current field work in Hospice.

Current clinical license in the state of Georgia.

Must have a minimum of one (1) year of professional experience in a home care or Hospice setting, or a clinical healthcare setting.

Must have current Driver’s license and reliable transportation to and from work site.

Ability to consistently demonstrate a commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.
