Cape Cod Healthcare Overview Under the direction of the clinician, department supervisor or designee, provide clinical support to the MACC practice. Deliver excellent customer service to patients as part of a medical care team. Maintain patient confidentiality according to CCHC and HIPAA. Responsibilities
Assist in providing clinical care to office patients under the direct supervision of the clinician, department supervisor or designee. Manage clinicians’ daily schedule and patient flow to expedite care and maximize clinical time. Greet and escort patients to exam rooms and obtain relevant information and vital signs as indicated (e.g., height, weight, blood pressure, heart rate, respiratory rate, oxygen saturation). Input pertinent clinical information into the electronic health record timely and accurately; perform a complete medication reconciliation. Assist clinicians with clinical procedures and/or additional tests specific to the department; advise patients of preparation required for exams or additional testing; inform patients of visit status and any delays. Prepare and maintain exam rooms for patient arrivals; manage inventory of supplies and equipment; notify supervisor to order clinical supplies and equipment as appropriate. Under physician supervision, administer routine injections when delegated and document in real time and with accuracy. Ensure telephone calls are answered promptly and clinicians receive necessary information; triage calls per established protocols when clinicians are unavailable. Assist in resolving patient concerns as an advocate when appropriate; participate in practice and department meetings and trainings to promote quality patient care. Stay current with community resources and the broader healthcare system; engage in ongoing self-assessment of skills. Maintain patient rights and confidentiality obligations per CCHC and HIPAA; provide clerical/office support as needed (e.g., scheduling, greeting/registering patients, scanning records, answering calls, etc.). Ability to float to other MACC practices when required; perform other duties as assigned. Assist with assessment of patient needs and coordinate care under the clinician’s direction; verify and implement clinician orders for testing and/or procedures (e.g., EKGs, dressing changes, spirometry, Holter monitors). Provide effective, evidence-based patient and family teaching, considering patient values, beliefs, learning needs, language, culture and socioeconomic status. Triages walk-in patients per established protocols and maintain communication for safe transitions and continuity of care. Education/Experience/Training
LPN (p015) – Education/Experience/Training Ability to read, write and communicate in English Graduate of an accredited Practical Nursing program Currently licensed as a Licensed Practical Nurse in the state of Massachusetts Basic Life Support Certification for Health Care Providers Strong interpersonal, customer service, and time management skills Experience with current office technology and proficiency with computer office applications (EMR, MS Office, etc.) Minimum of two years’ experience in a clinical setting preferred RN (x21a) – Education/Experience/Training Ability to read, write and communicate in English Graduate of an accredited school of nursing Currently licensed as a Registered Nurse in the Commonwealth of Massachusetts Basic Life Support Certification for Health Care Providers Experience with current office technology and proficiency with computer office applications (EMR, MS Office, etc.) Minimum of two years’ experience in a clinical setting preferred Schedule Details
24 hours per week, Monday–Friday, 8a–4p; no weekends and no holidays. Sign-on bonus eligible. Organization
