A New Leaf is seeking a dedicated

Tenancy Support Specialist

to join our Intensive Case Management Services Program. This role provides vital support to individuals and families who are at risk of homelessness or facing housing instability. The Tenancy Support Specialist will assist clients in securing and maintaining stable housing by providing case management, tenant education, and resource coordination. This position is responsible for empowering clients with the knowledge and skills to successfully navigate the housing system, ensuring lease compliance, and fostering positive relationships with landlords. The ideal candidate will have strong communication skills, a deep understanding of housing programs, and a passion for helping vulnerable populations achieve long-term housing stability.

Hourly Pay:

$25

Schedule:

Monday – Friday; 8am – 5pm (flexibility needed to meet client needs)

Maricopa County, AZ

QUALIFICATIONS Education:

High School Diploma/GED or equivalent required.

Experience:

1-2 years of direct case management experience, required.

Experience working with various homeless populations, including families, individuals, young adults and/or homeless veterans.

DUTIES AND RESPONSIBLITIES This position must use a personal vehicle and transport participants and may attend meetings and appointments as they relate to housing search and support services.

This position plays a critical role in increasing the positive health and wellbeing outcomes for the populations served in the AHCCCS H20 program and is integral in helping people maintain housing stability. This position supports participants with securing and sustaining housing.

Responsibilities will include but are not limited to:

Provide pre-tenancy and tenancy supports to a caseload of 1:15 based on the H20 program requirements.

Provide ongoing connection to resources and housing stabilization supports to the Seriously Mentally Ill (SMI) designated participants referred by Solari.

Partner with the participant’s Health Home, Health Plan, and Network Administrators (Solari/Banner) and other Housing Partners throughout the delivery of H20 support services.

Provide participants advocacy and support with locating affordable units in the community to include other housing subsidy intervention as appropriate.

Check in with participants as needed, at a minimum of 4 times per week, as indicated by AHCCCS H20 requirements.

Complete timely documentation of referrals, case notes, assessments, and various reports in required timeframes to ensure proper billing.

Navigate and utilize multiple platforms and systems for various documentation to include HMIS, Community Cares, Unite Us, and any other database required for the H20 initiative.

Provide individualized support by helping each participant implement a Housing Care Plan that addresses their barriers, increase their income, and maintain and sustain permanent housing.

Provide advocacy, case management, benefit establishment, linkage to Mental Health/Substance.

Provide services to participants in the areas of independent living skills, employment/job assistance, benefits acquisition, and other services as needed to assist in maintaining housing.

Provide participants with an appropriate level of assistance with locating, applying for, and moving into new units, and provide assistance with setting up utilities when needed and providing donations and start up items when necessary.
