The Case Manager, East Mountain Youth Lodge (EMYL) at Carrier Clinic provides administrative case management support to the clinical and leadership teams of EMYL in maintaining the proper clinical documentation, various signed consents, and psycho‑educational plans for residents of EMYL. This administrative case management support will help ensure compliance with the New Jersey Department of Children and Families (DCF), Division of Children’s System of Care (CSOC) licensing standards for Intensive Residential Treatment Services (IRTS), Residential Treatment Center & Behavioral Health/Substance Use (RTC & RTC/BH/SU), and Psychiatric Community Home (PCH) programs at EMYL.
Responsibilities
Coordinate daily with leaders in assigned areas for daily assignments and carry out standing duties.
Communicate with internal departments and outside treatment partners and families.
Complete specific duties according to assigned role, including:
Admissions Case Management Responsibilities
Schedule meet and greets for new residents and families.
Answer phone calls in real time.
Facilitate pre‑admission processes and paperwork.
Facilitate admissions into the East Mt. Youth Lodge.
Nursing Case Management Responsibilities
Insurance verifications/referrals.
Coordinate vaccine appointments and tracking for residents.
Assist with scheduling resident lab testing needs.
Guardian outreach for medication consents.
Assist with management of nursing groups.
Medication room audit.
Schedule routine appointments.
Clinical Case Management Responsibilities
Verify consents at intake and obtain new consents.
Manage discharge planning paperwork.
Coordinate meet and greets.
Receive and process paperwork for next placements.
Communicate with internal and external teams about planning.
Act as school coordinator: calls for SCESC, process intake paperwork from guardians, work on communication with districts and SCESC for confirmed start dates, attend meetings with EMS regarding classified pending students.
Adolescent Unit communication.
Email phone list upon admission, send AU letters to OOL, provide school re‑entry form with clinical approval.
Team phone calls and updates, scheduling treatment meetings.
Treatment planning: attend Joint Care Review (JCR) treatment meetings, start JCR and CANS with final approval from therapist, update phone/visit/pass plans with clinical leadership approval, label, file, compose mailing letters/emails, schedule visits/passes with CMO and guardians.
Attend leadership meetings for relevant input and follow‑up related to care of residents and administrative case management support.
Other duties and/or projects as assigned.
Adhere to HMH Organizational competencies and standards of behavior.
Qualifications
Bachelor’s degree in relevant field with minimum of 3 years of relevant experience or Master’s Degree with 1 year of relevant experience.
AHA HeartSaver First Aid CPR AED Certification, or completed within 90 days of employment.
Compensation and Benefits Starting hourly rate: $25.38. Compensation determined by experience, education, certifications, market data, and internal equity. Additional incentives may apply.
