The Center is a 501(c)(3) nonprofit organization that assists people regardless of religious affiliation through integrated social service programs. It enjoys broad community support and a sponsored relationship with the Sisters of St. Joseph of Carondelet. Brief Description of Principal Activities The Center conducts intervention, prevention, and education services across four sites on the Westside of Los Angeles, providing clients with coordinated access to services based on their needs. Job Description

The Case Manager is part of a team of social service professionals ensuring quality care delivery for contracted agencies, aligned with the mission of St. Joseph Center. Key Responsibilities

Provide ongoing case management to help individuals and families retain housing and be good tenants and neighbors. Make referrals and link clients to programs to increase self-sufficiency. Develop relationships with outreach workers, police, housing authorities, and partner agencies. Essential Duties

Conduct mental health/substance abuse screenings and housing needs assessments. Create and implement individualized care plans addressing housing, mental health, substance abuse, relapse prevention, medication management, and other risk factors. Assess client progress regularly. Maintain organized client files and update electronic records. Utilize evidence-based models like Motivational Interviewing, Harm Reduction, Housing First, and Trauma-Informed Care. Provide services that promote permanent housing, social integration, and prevent relapse, including advocacy and linkage to services. Assist clients with benefits, housing applications, and life skills such as budgeting and accessing resources. Offer crisis management and substance abuse/mental health services in various settings. Respond promptly to urgent client or landlord requests. Participate in case conferences and community meetings. Qualifications

Knowledge, Skills & Abilities:

Highly motivated self-starter with excellent communication, organizational, and administrative skills. Knowledge of substance abuse, homeless programs, mental health, and intervention techniques for hard-to-reach populations. Bilingual skills are a plus. Ability to work flexible hours, including early mornings and evenings. Experience:

At least one year of experience working with individuals with severe mental illness or co-occurring disorders, including crisis intervention and referrals. Education:

Bachelor’s degree in social work, human services, or a related field, or two years of relevant case management experience. Other Requirements:

No supervisory responsibilities. Proficiency in MS Office and electronic health records. Valid California driver’s license and insurance. Ability to travel as needed. Work Environment & Physical Demands

The work environment is moderate noise. Physical demands include standing, sitting, walking, reaching, and lifting up to 20 lbs., with accommodations available as needed. Additional Information
