Responsibilities As a Clinical Navigator, you will serve as the hospital’s face, meeting patients at their most challenging moments and guiding them toward success during their rehabilitation stay. The position reports to the Director of Business Development and includes:

Developing referral relationships and managing your assigned territory.

Conducting patient assessments and analyzing referral and admission data.

Collaborating with admissions, nursing, and other internal and external staff to facilitate referral conversion.

Providing feedback and recommendations for program and quality improvement related to customer service, referral processes, and patient satisfaction.

Establishing and maintaining relationships with stakeholders including families, physicians, ICU and floor nurses, discharge planners, case managers, and payer representatives.

Qualifications

Education and Training : Degree in a health‑related field from an accredited college or university preferred. Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required. Prior marketing and/or rehabilitation/LTACH experience preferred.

Experience : Minimum of two years’ experience in the designated licensed field, preferably with rehabilitation/LTACH and marketing experience.
