Clinical Manager of Integrated Care, LISW/LSW/LPC/LPCC
Zepf Center has been serving the Lucas County community for over 50 years and is the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, therapy programs, career development, wellness services, and primary care medical services to ensure a continuum of care. We operate as a trauma‑informed agency.
Hours:
Monday – Friday, 8:30 am – 4:30 pm
Essential Duties and Responsibilities
Monitors provision of Adult Mental Health Case Management services and other assigned clinical programs, ensuring they meet and exceed Fidelity, ODMH, and CARF requirements.
Oversees all care coordination areas of comprehensive care management, care coordination, health promotion, transitional care and follow up, individual and family supports, and referrals to community support services.
Makes and receives referrals, processes case assignments, ensures staff deliver high‑quality services.
Provides direct supervision to assigned Case Coordination teams at various Zepf locations if necessary.
Oversees orientation and training of new employees, ensuring all onboarding paperwork is complete and submitted timely.
Works with the Chief to determine, establish, and maintain appropriate staffing levels for facility and program coverage.
Develops agenda and conducts team meetings.
Be eligible to become a Health Officer and provide risk assessments in the community.
Manages day‑to‑day operations of assigned programs including budget formation and maintenance, purchase requisitions, and contract adherence.
Develops, implements, evaluates and enhances Adult Mental Health Case Management services and other assigned clinical programs to meet consumer, family, funding, and stakeholder needs.
Builds effective relationships with community partners to enhance overall client health. Processes case assignments, transfers, terminations, and assures staff deliver high‑quality services.
Monitors and facilitates consumer identification and engagement, completion of comprehensive health and risk assessments.
Oversees the team in development and monitoring of person‑centered care plans and progress toward outcomes.
Acts as agency representative of Adult Mental Health Care Management and other programs to the broader health care community, including Medicaid Managed Care Plans, hospitals, and service organizations.
Fosters program development through ongoing marketing initiatives.
Coordinates or facilitates handoffs and case consultations among providers.
May provide individual, group, and family prevention and care coordination services both in agency facilities and in the community based on client needs.
Ensures compliance with all documentation and productivity standards, certifications, and licensing standards.
Provides on‑call supervisory support and on‑site coverage for the Client Services Division as required; coordinates with psychiatrist, nursing, and other health care providers regarding services.
Performs other duties as assigned.

Master’s degree from an accredited college or university in social work, psychology, or a closely related human service field (preferred).
Ohio LSW, LPC, or RN license required. Preference given to candidates with an Ohio LISW or LPCC license.
At least three years of behavioral health experience.
