Public Safety Responsibilities and Duties

General description: Performs routine (journey-level) case management work to develop and maintain contact with assigned youth, their families, and providers for medical, social, educational, and related service needs. Includes case file reviews, case planning, family involvement, group service delivery, goal development, and evaluation of outcomes. Performs discharge and release coordination to ensure service provision throughout the youths commitment to TJJD. Works under moderate supervision with limited latitude for initiative and independent judgment. May include overtime, varying shifts, and on-call duty. Essential duties include: conduct initial case assessment and file reviews, request missing documentation, perform stage assessments, ongoing caseload assessment, develop Case Plans, coordinate referrals (e.g., MHTP/MROP, TCOOMMI), monitor caseload, coordinate transition planning, lead groups, provide individual counseling, maintain documentation, review youth health and safety information, ensure compliance with policies and regulations, and participate in staff development and safety practices.
