Overview
Provides for the safety, stability and well-being of children and families during the referral period. Delivers culturally competent, strengths-based case management services to children and families consistent with the evidence-based practice models. Meets DCF timelines to connect with the family within 24 hours of referral; facilitate initial family meeting within 48 hours of referral; complete assessment-based case plan within 20 days of referral; and case closure within 3–6 months of referral. Engages and works in partnership with families to complete assessments and develop case plan goals and objectives. Meets with the family in the home weekly to monitor progress towards case plan goals, and meets monthly with each child individually to assess child safety. Full-Time
Responsibilities
Provide for the safety, stability and well-being of children and families during the referral period Deliver culturally competent, strengths-based case management services to children and families consistent with evidence-based practice models Meet DCF timelines to connect with the family within 24 hours of referral; facilitate initial family meeting within 48 hours of referral; complete assessment-based case plan within 20 days of referral; and case closure within 3–6 months of referral Engage and work in partnership with families to complete assessments and develop case plan goals and objectives Meet with the family in the home weekly to monitor progress towards case plan goals; and meet monthly with each child individually to assess child safety Assess each child’s safety with every family contact and report all suspected child abuse/neglect. Monitor family progress and service provision using assessment tools, and update the family case plan based on assessments Provide weekly home-based family skills training including nutrition, hygiene, household and time management, home and child safety, basic parenting, transportation, problem solving and communication Work closely with the Family Support Worker to provide services for the family and make decisions about child well-being
Requirements
Bachelor’s degree in human services required Minimum two years of relevant experience Must have lived experience with substance abuse, treatment and recovery with a minimum of two years of continuous recovery Knowledge of recovery resources and experience interacting with individuals seeking recovery Must possess a valid state Driver’s License, current automobile liability insurance, and maintain a Motor Vehicle Record (MVR) that meets the organization’s insurability standard; reliable transportation is required
Knowledge, Skills, and Abilities
Overall computer knowledge and proficiency with database programs, spreadsheets and word processing Ability to operate office equipment (computers, e-mail, copiers, fax machines, telephones, voicemail) Effective listening, oral and written communication skills, including face-to-face discussions Regular and predictable on-site attendance Proficiency with Microsoft Word, Outlook, Excel, and PowerPoint Ability to operate other office equipment such as scanners Adherence to organizational values Commitment to providing respectful, quality services to the people and communities served
