The Family Preservation Services Case Manager provides for the safety, stability and well‑being of children and families by and coordinating culturally competent case management services that are responsive to the unique needs of the family.

Essential Functions

Provide safety, stability and wellbeing of children and families during the referral period.

Deliver culturally competent, strengths‑based case management services to children and families consistent with evidence‑based practice models.

Make decisions on guidelines provided by the supervisor, family preservation standard procedures and policies, evidence‑based practice model and DCCCA organizational policies.

Meet DCF timelines: connect with family within 24 hours of referral; facilitate initial family meeting within 48 hours of referral; complete assessment‑based case plan within 20 days of referral; and case closure within 3–6 months of referral.

Engage and work in partnership with families to complete assessments and develop case plan goals and objectives.

Meet with the family in the home weekly, monitoring progress toward case plan goals; and meet monthly with each child individually to assess child safety.

Assess each child's safety with every family contact and report all suspected child abuse/neglect.

Monitor family progress and service provision using assessment tools, and update case plan based on those assessments.

Provide weekly home‑based family skills training including Nutrition, Hygiene, Household and Time Management, Home and Child Safety, Basic Parenting, Transportation, Problem Solving and Communication to assist parents in providing a safe, nurturing environment for children in their care.

Work closely with Family Support Worker to provide services for the family and make decisions about child wellbeing.

Prioritize identified problems that require resolution, establish goals and objectives, utilize available resources and arrive at decisions appropriate to the family.

Complete and analyze initial and ongoing family assessments related to safety, stability and wellbeing, including mental health, substance abuse, education, emotional, social and physical wellbeing, parenting and parent‑child relationships.

Utilize assessments to develop case plan in partnership with Family Support Worker/Family Mentor, child/ren and family; make community referrals as necessary.

Complete and submit timely court reports; appear and testify in court as required.

Collect drug screens as necessary; document case information weekly; respond promptly to family crisis situations; and be available 24/7 to clients.

Be able to regularly drive to various DCCCA locations and other business locations throughout the day or night as required.

Promote and practice teamwork in all activities; foster and maintain professional relationships with families, community partners and peers; participate in professional development relevant to the role.

Education, Experience, and Licensure

Bachelor's degree in human services is required.

Prior work in behavioral health or children/family services is required; personal experience with addictions treatment is preferred.

Must strong and written communication skills; demonstrate flexibility and adaptability; align with diverse cultures, people, and organizations.

Solid commitment to principles of support and advocacy for foster and adoptive families.

Must possess a valid state Driver's License, be in force automobile liability insurance, and maintain an MVR meeting DCCCA insurability standards. Reliable transportation required.

Knowledge, Skills, and Abilities

Overall computer knowledge and proficiency with database programs, spreadsheet programs, Word processing programs.

Proficiency operating office equipment (computerized e‑mail systems, calculators, copiers, fax machines, telephones, voicemail systems).

Effective listening, oral, and written communication skills, including face‑to‑face, on‑site, in‑person discussions; use positive language appropriately.

Regular and predictable on‑site attendance.

Proficiency with Microsoft Office suite (Word, Outlook, Excel, PowerPoint).
