Isaacs Neighborhood Center (Isaacs Center) are leading Manhattan community-based agencies serving approximately 30,000 New Yorkers. The Senate is a permanent supportive housing residence for formerly homeless adults, many with mental illness, substance use disorders, and co-morbid medical conditions. On-site services include medication monitoring, case management, money management, entitlement assistance, medical and psychiatric care, substance use counseling, work readiness training, 24/7 front desk security, and group activities. Purpose of Position:

Provide case management services to formerly homeless adults diagnosed with severe persistent mental illness to assist tenants in maintaining permanent housing, using a client-centered approach to support goal achievement. Schedule:

Sunday–Thursday, 9:00 am–5:00 pm Base Pay:

$45,730.13/yr – $57,162.66/yr Note:

Candidate qualifications influence salary offer based on years of experience in the exact or comparable role and additional preferred qualifications. Responsibilities

Provide case management services Develop and monitor short- and long-term treatment goals for a group of assigned clients and link to community programs (medical, psychiatric, substance use treatment) Coordinate and follow up with onsite medical and psychiatric treatment providers Provide onsite medication monitoring Accompany clients to doctors, entitlement offices, and other community services Maintain case records with documentation of services and outcomes per program procedures Assist clients with entitlements and with understanding and complying with building rules and government subsidies to maintain residency Collaborate with staff across GRCC programs to coordinate client services Encourage participation, conduct floor meetings, and plan activities Participate as a Senate team member; attend staff meetings, case conferences, trainings, and supervision Participate in planning and leading group activities Maintain an active resource directory Track and document tenant outcomes related to physical, mental, and behavioral health using a results-oriented framework Other duties as required, including unit inspections and reviewing changes in government policies, pharmaceuticals, and insurance Qualifications

Bachelor’s Degree in social work or a related field required Experience working with individuals diagnosed with severe and persistent mental illness, substance use disorders, and/or co-occurring conditions preferred Spanish speaking preferred Skills, Knowledge and Abilities

Understanding of the housing first model Understanding of severe and persistent mental illness Understanding of harm reduction and trauma-informed care Ability to multitask Strong crisis management skills Strong interpersonal, written, and verbal skills Team player Computer Skills

Proficient in Microsoft Outlook and Excel Able to use or learn databases such as AWARDS Physical Requirements

Some walking and travel to escort clients to appointments; occasional hospital emergency room visits Work Environment

Mostly indoor office; occasional kitchen and basement during groups; regular client apartment visits Benefits
