Case Manager role at Glove House, Inc. Location: Community Residence Hours: 37.5/1950; On Call: Yes; Eligible for Flex Schedule: Yes Summary
The Case Manager is a leadership position within the Community Residence partnering with the Community Residence Program Manager, Senior Counselors, Social Worker, and Nurse. As a co-leadership team, the role is responsible for the health, wellness, and care of youth in the program. Knowledge of childhood trauma and its impacts is paramount. The Case Manager will identify and assess client and family needs, coordinate and provide services and/or treatment, and provide in-home supportive services. They will complete required assessments and assist individuals and families by helping to articulate goals and providing needed information. Primary Job Functions
Provide Behavioral Health Overlay Services (BHOS) interventions under the supervision of a licensed clinician while upholding the Glove House Standard of Care. Guided by organizational values and trauma informed practice in all interactions. Demonstrate understanding of the four dimensions of safety and identify gaps. Treat all children, families, staff and ancillary providers with respect and courtesy. Demonstrate sensitivity to the cultural background of children, families, and co-workers. Ensure activities follow Glove House policies and procedures, OMH professional standards, ethical standards, accreditation standards, and applicable law. Interact with staff and youth daily to ensure services are delivered per the Glove House operating modality (TCI) and treatment plans. Assist in providing staff and resident counseling and support in crisis situations, including post-crisis response. Model positive behaviors and core values; assist with intake and pre-placement processes. Coordinate services between the Community Residence and clinic treatment programs or other community resources (e.g., schools, vocational, therapy). Develop relationships with community organizations to improve service access; maintain regular contact with families and guardians; arrange family visits and prepare for reunification. Coordinate transportation for home visits; ensure eligibility and payment from SSI, Medicaid, and other third-party funding sources. Facilitate psycho-educational classes/groups; train direct care staff on treatment plan components; participate in treatment team meetings. Complete administrative tasks; ensure adequate staff coverage; maintain client records and program documentation (self, staff, youth) including daily activities, treatment progress notes, and termination summaries according to standards; timely documentation (e.g., myEvolv). Participate in team activities, staff meetings, Glove House strategic planning, risk management, and Quality Assurance plans. Maintain compliance with Glove House standards and provide required documents (physical/TB, driver license, auto insurance). Protect client privacy in accordance with HIPAA privacy and security rules. Contribute to a positive work environment; travel on agency business with mileage documentation; attend mandated meetings and committees; represent Glove House at state/local forums. Perform other responsibilities as assigned by the Community Residence Program Manager. Travel may be required; safety and incident handling as applicable. Qualifications
Education, Licenses, and Certifications
All degrees from an accredited university. Bachelor’s degree in the human services field; master’s degree preferred. Valid driver’s license and acceptable driving record. Minimum 2+ years of experience working with children and families. Knowledge, Skills, and Abilities
Knowledge of community resources, TCI Principles, Trauma-Informed Care Practices, and state/federal regulations. Ability to perform under demanding, fast-paced conditions; handle confidential information appropriately; work autonomously with minimal supervision. Multi-task with attention to detail and quality; commit to high levels of customer service and positive outcomes. Demonstrate Glove House values; interact effectively with diverse individuals and families; set boundaries; protect clients when necessary. Assess and apply appropriate intervention strategies; understand environmental stressors and social/economic factors affecting families. Be culturally sensitive; maintain home visit readiness where applicable. Schedule & Work Environment
Standard week with potential weekends, holidays, and evenings. Flexible hours and on-call support as needed. Travel as required; approximately 80% on-site and 20% remote. Work in a residential care facility with possible exposure to illnesses and/or uncontrolled situations.
