Position Summary Page Health & Rehabilitation Center in Fort Myers is seeking a skilled and compassionate Physical Therapist (PT) to join our in-house subacute rehabilitation team. As a key member of our rehab department, the PT will evaluate, plan, and implement individualized treatment programs to help patients regain mobility, improve function, and achieve their highest level of independence. This role offers the opportunity to work in a supportive, patient‑centered environment with flexible scheduling.
Key Responsibilities
Perform comprehensive evaluations and develop individualized care plans based on clinical findings and patient goals
Provide evidence‑based physical therapy treatments to restore function, relieve pain, and prevent disability
Supervise and delegate appropriate treatment interventions to Physical Therapist Assistants (PTAs)
Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes
Educate patients, families, and caregivers on therapeutic exercises, functional mobility, and safe discharge practices
Accurately document assessments, treatments, and progress in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams, including nursing, occupational therapy, and speech therapy, to ensure coordinated patient care
Maintain a safe and clean treatment environment and ensure equipment is used properly
Qualifications
Graduate of an accredited Physical Therapy program
Current Florida license as a Physical Therapist
CPR/BLS certification (or ability to obtain prior to start)
Experience in a subacute rehab or skilled nursing facility preferred; new graduates are welcome
Strong clinical reasoning, communication, and documentation skills
Commitment to providing compassionate, high‑quality care
Ability to adapt to changing patient needs and prioritize workload efficiently
