The Assessment Nurse may be the first point of contact for potential members and their families in the MLTCP programs. The role involves conducting UAS assessments to determine the level of care needed to support applicants/members in their homes. The Assessment Nurse also completes assessments for current members upon enrollment, semi-annually, and after significant changes in condition. Job Responsibilities
Meet with applicants, families, and caregivers in person to assess eligibility for the MLTC program based on clinical qualifications. Conduct in-person assessments for enrollees and prospective enrollees using the UAS tool. Document outreach attempts in the care management data system. Perform at least 13 UAS assessments weekly, including initial, change in condition, return, and re-assessments, with at least 60% in-person. Gather information from members and other sources like DCMS to complete the UAS. Conduct
