Evaluates, plans, and implements goal-oriented Physical Therapy treatment programs focused on restoring function and preventing disability due to trauma, disease, surgery, or other conditions. Responsibilities
Participates on committees as requested. Delivers high-quality customer service to residents, families, and visitors. Models and maintains a safe environment, employing correct body mechanics and appropriate treatment techniques. Develops discharge plans, completes summaries, and updates goals throughout treatment. Assesses need for DME, ensures proper fitting, and educates residents and families. Prepares documentation such as progress notes, re-evaluations, and discharge summaries timely and accurately. Gathers evaluation data, including testing and consultations, respecting departmental policies. Develops rapport with residents and team members, providing education and fostering goal achievement. Implements individualized treatment plans, develops goals, and manages assistive devices and equipment. Participates in discharge planning and ensures proper utilization of devices. Handles special projects and urgent assignments as needed. Adheres to organizational competencies and standards of behavior. Qualifications
Graduate of an APTA-accredited program or equivalent. Physical ability to perform required tasks, including lifting and moving heavy objects. Excellent communication skills and proficiency with computers. Licenses and Certifications
Valid Physical Therapist License. AHA Basic Life Support (BLS) Certification. Contacts
