Job Summary And Qualifications The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The PT completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The PT provides or directs a majority of the patient’s physical therapy to support patients throughout the continuum of care.
What you will do in this role
Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
Prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.
What qualifications you will need
Graduate of an American Physical Therapy Association accredited school with a BS, MS, or Doctorate degree in Physical Therapy.
Active Physical Therapy license in the State of Colorado.
Current certification in BLS for HealthCare Providers (American Heart Association guidelines).
Successful completion of student clinical rotation or work experience in at least one of the following settings: acute care hospital, long-term acute care facility, skilled nursing facility, and/or acute rehabilitation setting.
Basic Cardiac Life Support obtained within 30 days of employment start date.
LPT – Licensed Physical Therapist.
Bachelor's Degree.
