Physical Therapist – Join the Soundview Team at HYATT FAMILY FACILITIES LLC. Location: Soundview Care & Rehabilitation. Schedule: Part-time – Up to 24 hours a week. Position: Physical Therapist. Are you a compassionate Physical Therapist looking to help seniors regain mobility and improve quality of life? Soundview Care is seeking a dedicated Physical Therapist to support residents' rehabilitation goals and daily functioning.

Responsibilities
Conduct comprehensive assessments of residents’ physical abilities and needs.
Develop and implement personalized treatment plans to enhance mobility, strength, and overall function.
Provide hands-on therapeutic interventions, including exercises, manual therapy, and education.
Collaborate with interdisciplinary team members to ensure coordinated care for residents.
Educate residents and their families on treatment plans, exercises, and strategies for improving mobility.
Document progress and outcomes effectively following regulatory standards.
Support residents in achieving their recovery goals.
On-the-job training and development opportunities.
Qualifications
Doctorate or Master’s degree in Physical Therapy from an accredited institution.
Active state licensure as a Physical Therapist.
Experience in geriatric care or a nursing home setting is preferred but not required.
Strong interpersonal and communication skills.
Compassionate approach and enthusiasm for working with the elderly.
Must be able to pass a background check and meet all state requirements
