Greenfield Rehabilitation Agency is seeking a PRN to Part-Time Physical Therapist (PT) to join our expanding therapy team at our newly acquired senior campuses in Pigeon Falls and Whitehall, WI. Evaluate and treat patients in a skilled nursing setting Develop individualized care plans focused on improving mobility, strength, and function Collaborate with the interdisciplinary team to ensure continuity of care Document patient progress in accordance with regulatory and company standards What You Bring
Active Wisconsin Physical Therapist license Strong clinical, communication, and organizational skills Passion for working with older adults in long-term care settings Dependability, flexibility, and a team-player mindset What We Offer
