Job Summary The Clinical Coordinator - Ambulatory Services supports a specialty population and/or group of specialty patients by coordinating patient care at the local level, across sites and settings, and/or for the duration of the care continuum. The Clinical Coordinator facilitates the development and provisioning of clinical resources; assists with evidence-based practice & quality improvement initiatives and facilitating specialty training and orientation. Required skills include critical thinking, excellent verbal and written communication, the ability to work autonomously/be self-directed, collaborating with others, acting as a change agent, and ability to teach peers. The Clinical Coordinator is able to complete all duties of the Staff RN role in their designated care area.

In addition, this nurse works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family center‑ed care. This role systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. The nurse utilizes evidence to drive clinical decisions and practice related to individual and population health. The nurse establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The nurse considers factors related to safety, effectiveness and fiscal accountability in planning and delivering care. The nurse delegates and supervises care in accordance with the WI Nurse Practice. This role participates in continuous quality improvement initiatives and assumes responsibility for their professional development.

Experience Description Three years as a nurse in a healthcare setting required. Experience in the specialty strongly preferred.

Education Description Nursing Licensure Required. Bachelor's Degree In Nursing, Preferred.

Licensure Description Requires current state of Wisconsin Registered Nurse License or Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days of hire.
