Alvernon Way, Tucson, AZ 85711 USA The primary purpose of this position is to deliver culturally responsive and linguistically appropriate health promotion services to Refugees and other ORR-eligible individuals in Arizona. This includes providing targeted health outreach and coordinated healthcare navigation services to at-risk individuals to ensure they can access and utilize complex medical and behavioral health systems. The role focuses on reducing barriers to care by supporting individuals through case management, education, and advocacy. This position helps improve health outcomes by empowering individuals to engage with and maintain access to essential health services. click here . Duties & Responsibilities

Healthcare Navigation and Case Management: Provide direct support to patients and families in our family medicine practice by delivering culturally responsive healthcare navigation and case management services including accessing medical and social services and benefits. Collaborate with family medicine faculty and residents and the clinic’s interprofessional Refugee Health team to ensure coordinated care and follow up. This will include offering targeted education, advocacy, and outreach support to help individuals access and utilize complex health and behavioral health systems. Data Tracking, Reporting, and Quality Improvement: Track client services and outcomes, report to grant funders, and assist with program evaluation. Collect and analyze data to support continuous quality improvement and contribute to academic research and grant-related projects. Community Partnership and Collaboration: Build and maintain partnerships with community service providers to enhance resource networks for refugee and ORR-eligible individuals. Attend regular meetings with the funder and other refugee health promotion providers across the state to collaborate on best practices, align services, and strengthen coordinated efforts. Knowledge, Skills & Abilities: Familiarity with the U.S. healthcare and behavioral health systems, including how to access and coordinate care across providers. Knowledge of public benefit systems such as Medicaid (AHCCCS), SNAP, and other state/federal assistance programs. Understanding of trauma-informed and culturally responsive approaches to health promotion and case management. Awareness of social determinants of health and how they impact vulnerable populations. Skilled in working with professional interpreters to ensure effective and respectful communication. Strong communication skills with clients, medical professionals, and community partners. Competent in using electronic case management systems, maintaining accurate documentation, and tracking services for reporting. Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and virtual communication platforms. Ability to establish trust and build rapport with clients from diverse cultural and linguistic backgrounds. Ability to coordinate multiple tasks and manage a caseload independently while meeting deadlines and documentation requirements. Ability to work collaboratively within an interprofessional team, including healthcare providers, attorneys, mental health professionals, and community partners. Ability to advocate effectively on behalf of clients within healthcare, social service, and public assistance systems. Ability to maintain client confidentiality and comply with HIPAA and agency policies. This job posting reflects the general nature and level of work expected of the selected candidate(s). Bachelor's degree or equivalent advanced learning attained through professional level experience. Minimum of 3 years of relevant work experience. Preferred Qualifications
