Case Manager – County Youth Program
Full‑time, 40 hours per week position providing case management and essential life skills group counseling to children and youth ages 12‑19 at the Saint Louis County Center for Youth on the Rise in Spanish Lake. The role includes establishing and maintaining relationships with local communities, businesses, nonprofits, and other stakeholders to best serve the youth and raise awareness of the youth center.
Responsibilities
Community Case Manager – Primarily out in the community, recruiting, and serving youth where they are, including schools, homes, court, doctors’ appointments, job interviews, etc. Conduct one‑on‑one sessions with youth and families. Facilitate life‑skill groups at the center and out in the community.
Community Case Manager – Hybrid Case Management (50%) and Out in the community 50% of the time. Includes recruiting, transporting youth to appointments, conducting one‑on‑one sessions, facilitating life‑skill groups, entering and maintaining accurate data in the program’s case management or data tracking systems, tracking and analyzing program metrics, generating reports for leadership, processing program billing, assisting in quality assurance reviews, and collaborating on continuous improvements.
Salary & Benefits Starting salary $41,600 to $49,920 annually. Benefits include affordable health insurance, dental and vision coverage, 4 weeks of paid time off in the first year, 11 paid holidays, a deferred retirement plan, paid life insurance, short‑ and long‑term disability, and more.
Duties
Recruit middle and high school youth participants, including those who have dropped out of high school.
Implement special outreach efforts to recruit at‑risk youth.
Conduct risk assessments.
Create Individual Care Plans for youth participants.
Market program services and initiate outreach to parents.
Provide crisis intervention and assess client risk for harm to self and others.
Develop and maintain a network of community contacts and resources for client access to opportunities and referral services.
Provide non‑therapeutic counseling and case coordination within the youth center treatment team.
Identify common needs of current and new client populations.
Design and implement innovative life‑skills activities and group counseling.
Advocate and arrange referrals of clients to community services.
Assist with weekly oversight and programming for youth in the Drop‑in Center.
Organize positive youth development activities.
Support participants in employment activities, including application assistance and resume building.
Facilitate life‑skill groups and youth development activities during drop‑in hours and in the community.
Coordinate center programming for youth pursuing their GED/High School Equivalency examinations.
Additional duties as assigned.
Minimum Qualifications
Bachelor’s degree in social work or a related field.
Other collegiate degrees coupled with commensurate experience will be considered.
Minimum one year of experience working with youth and providing case management, group facilitation, and program development.
Strong interpersonal and communication skills; ability to build rapport with diverse youth populations.
Must have a valid driver’s license.
Hybrid Case Manager Only
Strong data entry, analysis, and reporting skills; proficiency in Excel and case management databases.
Excellent organizational and time‑management skills with the ability to balance field work and administrative tasks.
Preferred Qualifications
Master’s degree in social work or a related field.
Licensed in Missouri or enrolled in a process leading to Missouri licensure (LCSW, LPC, Psychology, Guidance Counseling, or a related profession).
Selection & Appointment A selection committee will evaluate the qualifications of each applicant, including education, experience, supplemental questions, and references. Candidates must earn a qualifying grade of 70 or more on the interview to be eligible for appointment. Selected candidates will be certified to the Director/Appointing Authority. All appointed candidates will be required to submit to a drug‑use screening prior to appointment.
