Medical Assistant_Care Coordinator - Population Health Location: Rockport, ME Facility: Waldo Hospital Schedule: Full Time Shift: Day Job Category: Nursing/Clinical Support Req #: 44596 Date Posted: Nov 3, 2025 0-1 years of experience eligible for $4,000 (full time) 1+ years of experience eligible for $5,000 (full time) Eligible candidates are hired (offer accepted) into a Full or Part time MA position. Bonus amount prorated for Part time hires; per diem hires are ineligible. Current MaineHealth member employees are ineligible; former MaineHealth Members are ineligible until greater than 6 months separation from employment. As a Medical Assistant – Care Coordinator, you’ll be the bridge between patients, providers, and the care they need. In this role, you’ll play a key part in keeping patients supported, informed, and on track with their health journey. From outreach and follow-up to helping guide smooth care transitions, you’ll make a real impact by improving the patient experience, supporting quality care, and strengthening our primary care team. Responsibilities
Required Minimum Knowledge, Skills, and Abilities (KSAs) as listed below: Education: Medical Assistant training preferred. License/Certifications: Certified Medical Assistant preferred. Current BLS Certification required or must obtain within 30 days of start date. Experience: Experience working in a medical office required. Additional Skills/Requirements Required: N/A Additional Skills/Requirements Preferred: N/A Qualifications
Education and certification requirements are described above. Experience in a medical office is required as noted in KSAs. Benefits
