Provide high-quality physical therapy services to patients in various settings. Develop personalized treatment plans based on patient assessments and goals. Achieve performance targets and contribute to team success metrics. Monitor patient progress and adjust treatment plans as necessary. Participate in ongoing professional development and training opportunities. Collaborate with healthcare professionals to ensure comprehensive patient care. Maintain accurate and timely documentation of patient treatments and outcomes. Utilize evidence-based practices to enhance treatment effectiveness. Educate patients and families on treatment plans and home exercises. Engage in self-reflection to identify areas for personal growth. Participate in team meetings to discuss patient care strategies and outcomes. Stay updated on industry trends and advancements in physical therapy. Experience/Qualifications
Valid state license for physical therapy practice in New Hampshire. Commitment to ongoing professional development and training opportunities. Experience in developing personalized treatment plans for diverse patient populations. Ability to monitor patient progress and adapt treatment plans accordingly. Strong communication skills to educate patients and families effectively. Familiarity with evidence-based practices in physical therapy. Proficient in maintaining accurate documentation of patient treatments. Team player with a focus on achieving performance targets. Willingness to stay updated on industry trends and advancements.
