Job Description
Dalhart Senior Care Solutions is an Outpatient Behavioral Health Clinic for older adults and seniors, a subsidiary of the Dallam Hartley Counties Hospital District. Therapists are to conduct initial assessments determining whether a patient is fit for the program. Therapists are responsible for providing a therapeutic group and individual setting for clients with behavioral and mental health needs. Therapists are responsible for documenting patient progress as well as any relevant treatment notes.
DUTIES and RESPONSIBILITIES:
Complete Biopsychosocial Integrated Assessment I & II, Social History, Cognitive Assessments, and Depression Scale with referred and current patients.
Discuss appropriate referrals with the program director and prepare patient for admission to the program back on the Diagnostic and Statistical Manual of Mental Disorders IV and V which includes the therapist’s impression of possible diagnosis with psychiatrist’s supervision and determination as a team.
Prepare and provide group psychotherapy why geriatric population patients requiring mental health outpatient support or other ages as license allows.
Document therapeutic notes regarding daily group therapy patient care following session and as needed.
Complete individual and/or family therapy as needed by the patient.
Monthly updated progress reports to primary care physicians/and or other agencies who are involved with patient upon signed release form.
Participate in treatment team planning meetings.
Communicate with telemedicine psychiatrist regarding patient treatment.
Complete master treatment plans with short term/long term goal setting for therapeutic process with each patient individually.
Complete treatment plan reviews during monthly treatment team meetings with telemedicine psychiatrist.
Prepare and provide individual/marriage/family therapy with behavioral health patients.
Discharge planning, referral to needed community resources for continued care upon discharge from structured outpatient program, SOP.
Marketing program to panhandle communities.
Offer mental health trainings to long term care facilities, community civic and religious groups to inform of mental health warning signs/need for individuals.
Assist with transporting patients when mental health tech needs assistance.
JOB QUALIFICATIONS:
Master’s Degree in Mental Health field and have obtained a Texas License as a Professional Counselor of LPC-A License.
Master’s Degree and have obtained a Temporary License Master Social Worker Texas License or Licensed Master’s Social Worker Level with intent to obtain supervision and become a Licensed Clinical Social Worker Level.
Previous experience in healthcare of behavioral health settings preferred.
