Responsibilities The Physical Therapist will primarily provide outpatient rehabilitation in an in‑home setting for older adults. Duties include evaluating patients, developing and delivering individualized therapy programs, coordinating multidisciplinary care, supervising staff as required, and maintaining accurate documentation.

Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure and professional standards.

Adhere to department schedules, complete timely clinical and billing documentation, and participate in patient care meetings.

Refer to other disciplines and agencies for comprehensive patient needs.

Consult with the rehabilitation team to select appropriate therapeutic regimens.

Adapt program and/or plan of care to individual patient needs.

Maintain a safe environment and eliminate hazards.

Order and maintain necessary supplies and equipment.

Educate patients and caregivers in skills for independence and productivity.

Perform assessments, apply therapeutic techniques (e.g., therapeutic exercise, manual therapy, airway clearance), and document treatment plans.

Conduct functional training, balance and agility programs, and health‑wellness education.

Execute home and community mobility assessments with recommendations for safe mobility.

Perform routine reexaminations and modify plans as needed.

Qualifications

Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.

Master's degree in Physical Therapy, or a related field with a Bachelor's in Physical Therapy.

Minimum of 5 years of progressively responsible PT experience if holding a Bachelor's degree.

U.S. equivalent credential for degrees earned outside the United States.

Valid driver’s license or reliable transportation for patient visits.

Posted Salary Range: USD $61.00 - USD $61.00 /Hr.
