The Care Manager position focuses on providing person-centered services and comprehensive care management, including planning and coordinating physical, behavioral, and support services. The goal is to increase independent living skills and promote positive community involvement for enrolled participants. The Care Manager guides person-centered goals and facilitates connections to natural and formal support networks. This role involves ongoing professional development to enhance knowledge of community resources, support access to services and benefits, and improve treatment documentation and approaches. The position operates primarily in the community but also requires working at the agency office and virtually via telehealth. The Care Manager must be experienced in using electronic health records for daily documentation. Education & Experience
Master’s Degree in a human services-related field OR Bachelor’s degree in a human services-related field with 1 year of relevant experience OR High School Diploma or GED with a minimum of 3 years of experience working with the target population OR Certified Peer Specialist (CPS) or Certified Family Specialist (CFS) Demonstrated Success In The Following Areas
Managing time effectively to meet deadlines. Recognizing symptoms of mental illness and assessing suicide risk; responding appropriately. Good verbal and written communication skills. Bilingual (English/Spanish) preferred but not required. Physical Demands
May involve lifting up to 20 lbs. 80-90% of the time, standing, walking, kneeling, or reaching. Requires full body motion, fine motor skills, emotional stability, sufficient vision and hearing, and the ability to speak and write. A valid driver’s license is required for community work.
