Position Summary
Person in this position will be responsible for providing case management services including assessment of service needs, care, service planning, linkage and advocacy for client and family with internally and external services. Essential Job Functions include those listed below.
Essential Job Functions
Coordinate client and family care with the treatment team and with external providers;
Assessment and periodic assessment of individual needs for case management services
Development and periodic revision of case management plan that includes service activities
Conduct individual case management sessions;
Participate in case conferences;
Communication, coordination, referral, linkage, and related activities
Monitoring service delivery to ensure client access to necessary services
Patient advocacy, linkages to physical and mental health care, transportation, and retention in primary care
Perform activities on behalf of the client and family, including attendance at external appointments as needed;
Maintain client files according to company policy and contract requirements;
Develop and maintain working knowledge of available community resources and act as liaison.
Maintain relationships with referring and supporting agencies and represent the agency at various community meetings as requested;
Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; and
Perform any other duties as assigned.
Certification/Licensure
Not Applicable to this position
Education
Associate degree preferred.
Essential Qualifications
2 years of related work experience; and lived experience being impacted and affected by a behavioral health disorder and treatment directly or indirectly (e.g. immediate family member or caregiver).
Bilingual/bicultural (English/Spanish) preferred
Flexible schedule to include weekends or evenings depending on program need
Excellent communication skills;
Ability to exercise good judgment and discretion;
Ability to work well in a team environment;
Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality;
Professional appearance and demeanor;
Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required.
Ability to pass and obtain a successful criminal background check
During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
And CPR Certification, First Aid Certification, and an annual Tuberculosis test.
Working Conditions
Work is primarily performed in an office setting;
Some travel is required and some outdoor activities may be required.
Professional appearance and demeanor.
Work varied shifts, including weekends and holidays.
