Evening Shift RN or LPN

Job Summary As an LPN/RN at Emerald Nursing & Rehab Omaha, you will provide high‑quality, compassionate care to residents. You will assess patient needs, deliver direct nursing care, and collaborate with the multidisciplinary team to ensure optimal outcomes.
Location & Work Environment Omaha, NE – Skilled nursing facility. Reports to the Director of Nursing.
Responsibilities
Administer medications, wound care, and other treatments as prescribed.
Monitor vital signs, document progress, and report significant changes to the Charge Nurse or Director of Nursing.
Assist in developing and implementing individualized care plans tailored to each resident.
Maintain accurate documentation and communicate effectively with residents, families, and the healthcare team.
Ensure adherence to facility policies, infection control, and safety protocols.
Provide emotional support and education to residents and families.
Collaborate with RNs, CNAs, physicians, and therapists to deliver coordinated care.
Qualifications
Valid LPN/LVN/RN license in Omaha, NE.
Graduated from an accredited LPN/LVN program.
Preferred prior experience in a skilled‑nursing or similar setting.
Current CPR/BLS certification.
Excellent verbal and written communication skills.
Physical ability to lift and move patients, stand for extended periods.
