Mosaic Life Care provides inpatient rehabilitation/post‑acute care in St. Joseph, MO.
Therapy plays an essential role in recovery from a life‑altering injury or illness.
Our unit is fast‑paced yet fun, offering endless opportunities to learn and grow. Responsibilities
Assists with daily coordination of personnel and resources within the scope of assignment; acts as liaison between nursing and ancillary departments to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance. Coordinating staff scheduling, educational/development activities, and performance evaluations. Collaborating with physicians, nursing, pharmacy, and other healthcare team members to determine, coordinate, and provide medical nutrition therapy. Identifying and resolving issues affecting the delivery of clinical services. Maintaining constant communication between all nursing areas and the registration area to facilitate smooth patient throughput. Providing basic direct patient care as needed, including insertion of PICC lines, accessing Hickman catheters and port-a-cath lines, and responding to in‑hospital emergencies. Assisting physicians with procedures and reporting all incidents or unusual events to the appropriate supervisor. Providing moderate sedation when required and ensuring knowledge of policy and procedure. Practicing within the scope of RN, abiding by facility standards, and treating patients and families with respect and dignity. Essential Functions
Discharge planning: develops individualized instructions for patient transition from hospital to home or subsequent providers, reporting to the social work team. Required Skills
Critical thinking, decisive judgment, ability to work with minimal supervision, and functioning effectively in a stressful environment. Qualifications
Master’s degree (OT/Doctorate for PT) required. Must hold a license in the state of MO. Basic Life Support (BLS) required. Advanced Cardiac Life Support (ACLS) required. Why Join Us
