The ACT Case Manager will work under the supervision of the Team Leader/Program Manager, focusing on person-centered services to increase symptom management, independent living skills, and positive community involvement for enrolled participants.

Essential Functions

Assisting in the development of and functioning as the coordinator of the comprehensive treatment/service plan in a manner that is person-centered and meets both agency and regulatory expectations.

Providing ongoing interventions, education, resource linkage and skills coaching in individual or group format related to symptom management, independent living, community activity, interpersonal interaction and socialization, and daily living skills.

Assisting individuals in accessing resources and education related to basic needs such as benefits, food/nutritional support, financial support/education, transportation, and stable housing.

Assists and supports participants to have and effectively use a primary care physician, dentist, and other specialists as required.

Organizes and leads individual and group social and recreational activities to structure enrolled participants’ time, increase social experiences, and provide opportunities to practice social skills.

Demonstrating the ability to recognize and assess suicide potential and risk, and provide appropriate responses to the situation.

Supporting individuals served and their families in crisis situations.

Provides on-call crisis intervention as scheduled.

Plans, structures, and prompts social and leisure-time activities on evenings, weekends, and holidays as assigned.

Service Team Approach

Demonstrating the ability to recognize and identify symptoms of mental illness and substance use.

Participating as a member of a multi-disciplinary team approach to treatment and recovery.

Coordinating with both internal and external providers regarding service provision.

Outreach, Engagement, Follow-Up

Providing comprehensive outreach and follow up to all assigned individuals including contact attempts over the phone, in-person, in the home etc.

Demonstrating the ability to engage and develop rapport with individuals presenting with a variety of symptoms/presentations.

Cultural Competence and Diversity

Providing services that are person-centered.

Providing services that are culturally relevant.

Providing a non-judgmental, open, and welcoming approach to diverse populations.

Documentation

Documenting all services provided within expected time frames.

Documenting all services provided in a manner that meets service definitions, regulatory standards, and agency expectations.

Documenting all services provided in a manner that is person-centered and meets quality improvement goals.

Efficiency and Productivity

Demonstrating excellent organizational skills.

Demonstrating effective use of time management techniques.

Demonstrating the use of effective engagement skills.

Consistently meeting established productivity standards.

Staff Development Activities

Participating in and completing all required trainings in an accurate and timely manner.

Identifying and requesting training or skill enhancement opportunities.

Engaging in cross-training activities with other team members.

Attending and participating in supervision sessions and demonstrating the ability to apply supervisory recommendations to work performance.

Other Completes other administrative support duties and special projects as developed and assigned.

Education and Experience

Master’s Degree in a human services related field OR

Bachelor’s Degree in a human services related field and 1 year relevant experience working with the target population preferred OR

High School Diploma or GED with a minimum of 3 years of experience working with the target population OR

Certified Peer Specialist (CPS) or Certified Family Specialist (CFS)

EXPERIENCE: Dependent on type of education (see above). Entry-level position. Experience needs determined by hiring manager.

OTHER: Valid drivers’ license and meets the driving privileges guidelines established in HR Policy 12.6 “Vehicle Safety and Client Transportation.”

Knowledge and Skills

Ability to manage time to meet all deadlines.

Ability to recognize and identify symptoms of mental illness and suicide and risk potential and respond appropriately.

Good verbal and written skills.

Bilingual (English/Spanish) is preferred but not required.
