$500 SUMMARY:
PATH Behavioral Health is expanding and seeking individuals to join our dynamic team. We are dedicated to redefining the mental health care experience by offering clients a fully integrated system of care with zero barrier access. We aim to serve historically underserved communities and are growing across Ohio. The Case Manager will collaborate with nurses, nurse practitioners, case managers, and therapists within our one-stop agency to support clients and contribute to the mission of reshaping mental health services. Responsibilities
Act as a liaison between care team members and the client’s family. Arrange family visits, coordinate transportation, and ensure clients’ needs are met. Maintain clear and ongoing communication among the family, referral source, and PATH Behavioral Healthcare. Engage in a professional and collaborative case management process to support clients’ mental and physical health needs. Assess clients’ individual needs and develop personalized care plans. Coordinate and collaborate with other care team members to ensure seamless delivery of care and support. Monitor clients’ progress regularly and adjust care plans as needed to address evolving needs and promote outcomes. Advocate for clients to access a variety of case management resources to help overcome obstacles and achieve goals. Qualifications
Education:
High School Diploma or GED required; Bachelor’s degree preferred. Experience:
Minimum of 1 year of case management experience. Physical effort:
Ability to work under stressful conditions and irregular hours. Communication skills:
Excellent verbal and written communication; professional telephone and written interactions; strong grammar and writing. Interpersonal skills:
Ability to interact in a friendly, compassionate, and professional manner with colleagues, providers, clients, and families. Technical/motor skills:
Competency in EHR, telehealth, and other company software; fine dexterity for tasks. Background:
Must pass all federal and state background checks; verification of education and work history per policy. Benefits
