Rate:
Starting at $60/hour (Based on Experience) Hanford Post Acute is seeking a
Part-Time Physical Therapist
to join our dedicated rehabilitation team. This role is ideal for a clinically skilled PT who is passionate about delivering high-quality therapy services in a post-acute care setting. Responsibilities
Evaluate patients within 24 hours or 1 business day of physician referral; document cause for any delay Develop individualized treatment plans and obtain physician approval Deliver care as outlined in the treatment plan Supervise Physical Therapy Assistants in patient care and related activities in compliance with the state practice act Communicate with interdisciplinary team regarding patient progress and care planning Participate in patient care conferences, utilization reviews, and in-service training Maintain accurate and timely documentation of evaluations, daily notes, progress updates, and recertifications in accordance with PT Board and payer requirements Instruct caregivers and nursing staff on maintenance programs and post-discharge care Recommend and assist in ordering durable medical equipment to enhance patient mobility and independence Report and monitor maintenance needs of therapy equipment Support nursing with Restorative Aide training Supervisory Responsibilities
Supervise PT assistants, aides, and students as applicable Qualifications
Bachelor's degree in Physical Therapy required; Master's or Doctorate preferred Current California PT license in good standing CPR certification and ongoing CEUs as required by the state Strong documentation skills, preferably with experience in Casamba and PointClickCare Experience in SNF or post-acute care preferred Ability to work collaboratively and maintain positive relationships with residents, families, and staff Physical Demands
Frequent standing, walking, reaching, and lifting (up to 100 lbs occasionally) Occasional stooping, kneeling, or crawling Vision requirements include close, color, peripheral, and depth perception Work Environment
Low to moderate noise level typical of a clinical setting Reasonable accommodations available as needed Apply Now
