Mobile Crisis Clinician, Full Time, Third Shift – UC Health Location: Cincinnati, OH, United States Responsibilities

Engages in population appropriate communication. Has knowledge of growth and development milestones and tasks. Gives clear instructions to patients/family regarding treatment. Involves family/guardian in the assessment, initial treatment and continuing care of the patient. Identifies any physical limitations of the patient and deploys intervention when necessary. Recognizes and responds appropriately to patients/families with behavioral health problems. Interprets population related data and plans care appropriately. Identifies and responds appropriately to different needs resulting from, unique psychological needs or those associated with religious / cultural norms. Performs treatments, administers medication or operates equipment safely. Recognizes and responds to signs/symptoms of abuse or neglect. Engagement And Exploration

Engage patients and the families referred to Mobile Crisis in the helping process. Assess the psychosocial and make decisions regarding Mobile Crisis response and priority of the response. Identify barriers to treatment. Enhance patient’s health status, increase patient/family satisfaction and quality of life. Improve utilization of resources to ensure appropriate level of care in either community outpatient care, hospitalization, and criminal justice. Develop report with patient and family for ongoing support and in response to crisis in community. De-escalation of patients in crisis utilizing best practices including trauma informed care. Goal Development And Implementation

Develop a comprehensive assessment to determine the appropriate level of care in collaboration with the care team. Identify referrals needed to support staying in the community when possible. Implement a plan of care and monitor patient responses; determine if an Emergency Admission is necessary, in collaboration with the medical team and psychiatry services when admitted. Educate and provide counseling to patient, next of kin, and community regarding level of care decisions using evidence-based practices. Utilize community resources to avoid hospitalization and support stabilization in the community. Facilitate follow up in the community upon discharge from PES or inpatient stay. Initiate referrals as indicated. Provide education about treatment options and stabilization in the community for mental illness. Advocate, mediate and negotiate to formulate a cohesive plan to maintain and enhance patients’ health status and social supports. Collaborate with law enforcement and the criminal justice system as appropriate. Provide appropriate intervention to patient and family, including counseling, crisis management, mental illness supports and referrals. Address material and financial aspects, health care decision making, and psychosocial barriers. Education And Consultation

Support post-hospitalization/Emergency Psych visits. Provide patient and family with connections to community resources and services. Participate in process improvement to evaluate patient outcomes. Assess transportation needs and arrange as appropriate. Facilitate referrals to disease-specific agencies related to employment, financial resources, support services, and chemical dependency services. Competency

Participate in activities that support enhanced customer service and improved patient experience metrics. Attend staff meetings and mandatory departmental in-service trainings; maintain positive relationships with community agencies and services. Maintain timely, clear, and concise documentation; write reports as needed; respond to email requests; participate in quality and safety initiatives. Location

Cincinnati, OH, United States Qualifications

Minimum Required: Master's Degree in Social Work, Psychology, Counseling, or related field. Required: Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC). Complete required training and health officer certification per Ohio Revised Code for Mental Health, Addiction and Recovery Services Board of Hamilton County within one year. Preferred: Licensed Independent Social Worker, Licensed Clinical Professional Counselor (LCPC). Minimum Required: 1 - 2 Years equivalent experience. Preferred: 3 - 5 Years equivalent experience.
