Job Title: Outpatient Physical Therapist
Position Summary
This role is responsible for providing medically prescribed, goal-oriented physical therapy in a patient’s home, therapy office or select location based on treatment/patient needs. The role includes providing therapeutic treatments and participating in the development of patient physical therapy plans of care.
Principal Accountabilities
Perform initial and ongoing physical therapy evaluations and re-evaluations according to APTA standards of practice as well as state standards.
Develop a plan of care/treatment plan for each patient based on the initial evaluation and objective findings including short and long-term goals, keeping the patient and caregiver as the focus of care.
Implement treatment plans based on an established schedule and adjust this schedule and plan according to patient progress.
Document patient progress according to approved medical documentation and clinic documentation procedures.
Evaluate mobility, strength and current level of patient function and pain, while setting goals to improve these deficits. Ensuring that the goals are scaled to the patients abilities and are realistically attainable.
Report patient progress to patients, patient caregivers and healthcare professionals involved in ongoing treatment/care of patients.
Develop an individualized plan for patients following completion of their Plan of Treatment with appropriate support and follow-up services as needed.
Maintain professional and technical knowledge through attending educational workshops, reviewing professional publications, etc.
Qualifications
Minimum successful completion of Physical Therapy school. New grads will be considered.
Compassionate and able to deliver quality patient-centered care
Excellent problem solving, interpersonal and communication skills
Motivated and able be excel in a self-managed environment
Desire to work a flexible schedule based on patient needs
Ability to travel to patient homes or desired location for treatment
Additional skill sets appreciated include experience in Women's and/or Men’s health, dry needling and vestibular treatment.
Education: Bachelor’s degree in Physical Therapy from accredited physical therapy program
Certifications, Licenses and Registrations
Currently licensed Physical Therapist in the state of Wyoming.
Possesses and maintain a current CPR certification
Valid driver’s license
