Care Coordinators

to support youth at our Brooklyn Center. In this role, you will guide program enrollees and their caretakers (legal guardians) through the healthcare system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. CARE MANAGER SUPERVISOR, HEALTH SERVICES Hours:

35 hours, Non-Exempt Salary Range:

$44K-$49K annually Obtain required Care Management enrollment consents from the individual or legal guardian. Complete initial and ongoing needs assessments (Child and Adolescent Needs and Strengths; CANS) to determine the most appropriate level of care management. Manage the patient’s Individualized Plan of Care, including coordinating services, supporting adherence to treatment, and monitoring needs. Meet client contact requirements, including face-to-face visits based on acuity level. Ensure timely and accurate documentation in the Care Management Portal (MAPP) and Electronic Health Records (EHRs). Advocate for clients within the agency and with external service providers. Promote wellness and prevention by linking enrollees with resources and services. Educate children and caregivers on chronic condition care, immunizations, and preventive interventions. Assist clients in obtaining and maintaining public benefits such as Medicaid, SNAP, housing, and legal services. Communicate effectively with individuals and families, considering language, literacy, and cultural preferences. Conduct care planning meetings and serve as an interdisciplinary team member. Identify community resources and manage referrals, access, engagement, and follow-up. Participate in discharge planning during hospital admissions to ensure post-discharge services. Attend ongoing staff development trainings. Ensure periodic evaluations and follow-up for dental, vision, and hearing care per Medicaid EPSDT guidelines. Perform other duties as needed. Qualifications of Care Coordinator Education and/or credential requirements:

Bilingual Spanish/English required. Previous experience as a Case Planner, Care Coordinator, Case Manager, or Case Worker in a social service setting. Experience with Child Welfare, ACS, and Foster Care is helpful. Bilingual candidates are highly encouraged to apply. Experience required: Experience serving children and families in child welfare, developmental disabilities, mental health, healthcare, or preventive services. For high acuity enrollees, demonstrated knowledge of their needs through additional experience, education, or training. Extensive experience coordinating care for children with medical fragility. Experience providing service coordination, linkages, and referrals for community-based services.
