Job Description

The pharmacist dispenses prescribed drugs in a healthcare facility environment, transfers prescriptions, provides drug information in collaboration with other healthcare professionals, performs medication order entry into the computer, monitors patient profiles for appropriate medication use, oversees the work of the support staff and documents any relevant interventions. The position is expected to meet state certifications, requires familiarity with standards concepts, practices, and procedures, and relies on experience and judgement to plan and accomplish goals. Responsibilities

TECHNICAL TASKS Reviews and interprets patient prescriptions and profiles to prevent overdoses, incompatibilities, and toxicity and to ensure high quality pharmacy services and patient care. Accurately and efficiently processes orders/IV admixtures. Demonstrates understanding of formulary system and ensures compliance with the formulary. Documents all clinical interventions in the department approved tracking system. Supervises and reviews all functions performed by support personnel within the pharmacy. Removes all out-of-date medication from inventory immediately upon discovery and checks all patients on that medication to ensure none will be given. Maintains physical inventory to ensure adequate supply of medications to meet patient demands. Completes all assigned daily tasks, and assists others when time permits. Completes other duties as assigned. Available to fill in for staff pharmacists as needed. CRITICAL THINKING Responsible for clinical use and knowledge of pharmacokinetics and pharmacological effects of drugs and chemicals on the human body. Calculates dosing and provides clinical suggestions based on patient parameters such as age, organ function, and weight to optimize therapy. Advises other health care professionals regarding drug properties, effects, administration, stability and compatibility to ensure safe and effective patient care. Uses references and computer programs to accurately dose and understand pharmacokinetics of drugs dispensed by the pharmacy. Provides in-service education to health care professionals and participates in community education activities when directed. Records and reports data including interventions that improve patient care, adverse drug reactions, medication errors, and cost-effective therapeutic interchanges. Able to assess a situation and choose the appropriate course of action. Demonstrates the ability to communicate effectively with diverse people, considering any special needs. INTERPERSONAL RELATIONS Communicates with physicians and nursing staff/customers about availability, side effects, and potential drug incompatibilities. Engages with pharmacy practices through reviewing newsletters, departmental minutes, and communication with management. Effectively manages and directs support staff in their daily responsibilities. Expedites resolution of communication deficits with other health care professionals and co-workers. Qualifications

Retail experience and working knowledge of 340B and Patient Assistance programs. Bachelor's Degree Licensed as a Registered Pharmacist (state appropriate); BLS (Basic Life Support / CPR). Seniority level
