Requirements
Provider qualifications for a Substance Use Care Coordinator (must meet one of three options):
At least a bachelor’s degree in social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation or human services counseling and at least one year of substance use related direct experience or one year of clinical experience with co-occurring diagnoses.
Licensure by the Commonwealth as a registered nurse with at least one year of direct experience or clinical experience with co-occurring diagnoses.
Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC‑Assistant under supervision as defined in 18VAC115‑30‑10 et seq.
Required supervision: all Substance Care Coordinators must be under the general supervision of a buprenorphine waivered practitioner or Licensed Credentialed Treatment Professional in the OTP or preferred OBOT setting.
Valid/current driver’s license.
Ability to travel up to 15% may be required.
Responsibilities
Providing case management and care coordination services for patients enrolled in treatment, coordinating continuum of care activities and ensuring optimum utilization of resources.
Coordinating patient assessments, treatment planning, crisis intervention services, community resource linkages, service delivery monitoring, and assisting patients in obtaining necessary services.
Conducting individual and family educational sessions and engaging in duties related to coordinating treatment and discharge activities.
Coordinating, in a timely manner, activities relevant to the team.
Adhering to organizational policies, procedures, performance improvement initiatives, and maintaining confidentiality.
Attend team meetings and complete all training courses timely as required.
Other duties as assigned.
