Job Description Collaborate with interdisciplinary teams to organize, prepare, and deliver evidence-based clinical services across the continuum of care. Uphold ethical principles and professional standards in practice.
Perform clinical diagnoses and conduct comprehensive bio-psychosocial assessments to establish care plans.
Implement integrated care model treatment guidelines by providing approximately 6-8 sessions of short-term, focused treatment.
Qualifications
Master’s degree in social work or related field required.
Licenses: LCSW, LPC, LMFT, or LADC required.
Prior experience in a medical setting preferred.
Bilingual skills preferred.
